Our distinguished programs and services are designed in a way that enhances the lives of our patients and their families. We are the only community provider that cares for our patients through the entire continuum of care which includes (but is not limited to): home healthcare, palliative care, behavioral health as well as hospice services. SERVICES WE OFFER
We are a full spectrum HealthCare Services organization.
HOME HEALTH
Assistance to help recovery from any form of hospital treatment that requires monitoring after discharge.
HOSPICE
For patients who have terminal illness and a life expectancy of six months or less.
PALLIATIVE CARE
Provides symptomatic supportive care to patients in between doctor visits and helps patients navigate complex medical conditions in their home setting.
BEHAVIORAL HEALTH
Home behavioral health for patients experiencing late life depression and similar conditions.
CAREGIVING SERVICES
Keep your independence in a familiar setting as we are there to assist you when and where you need it most.
STAFFING
Come work with us! We offer positions in Medical Home Health, Travel Nursing, and in Facilities.
SCHOOL NURSING
We provide the timely staffing of health service professionals and program supports to K-12 school districts.
CASE MANAGEMENT
We offer a complete support system for people living with symptomatic HIV or AIDS.

"In February of this year I underwent surgery at UC Davis Medical Center in Sacramento. When I was discharged, I was taken to my daughter's house nearby and your agency was contacted to provide skilled nursing services as well as the additional services I required for a time. All of the individuals your agency sent to help me were excellent, but Dixie Miguel was outstanding. He was called on to dress a rather large surgical incision in my leg every other day; he monitored my tube feeding and taught my family how to ensure the feeding was going where it was supposed to go and not into my lungs; he redressed the the incision into my trachea, and taught me how to give myself twice daily anti-clot shots in my belly. He said I reminded him of his own mother, and he was kind, sweet, good natured, and consistently cheerful. There are not enough words to express how happy we were to see him when he arrived.
