Referrals for home health are made by physicians, hospital discharge planners, and other medical providers. You may choose your preferred home health agency. Care is provided in you home or place of residence. A nurse or therapist will make a throrough assessment of your health care needs, and coordinate a Plan of Care with your physician.
We take special care to support the patient, family, and physician in schieving continuity of care. We will work closely with other service providers to make sure all of your needs are met.
