The AIDS Project provides healthcare monitoring and coordination of key services to people with HIV/AIDS. These services will enhance the clients’ ability to remain independent and in their own homes during active stages of their illness.
• Have a diagnosis of disabling HIV or AIDS
• Currently have HIV/AIDS related symptoms
• Be under the care of a physician
• Reside in Sacramento or Yolo County
We take special care to support the patient, family, and physician in achieving continuity of care.
The case management program consists of caring and highly trained nurses and medical social workers who function as a team to assess the client’s medical and psychosocial needs
The services we provide are made possible through funding from the California Department of Health Services, Office of AIDS. Services are free of charge to all our clients regardless of financial status.
